"Necessity is the mother of invention." After giving private and group lessons for a few years I finally realized this simple worksheet. When a new student is acquired I meet with them, conduct a trite interview, and offer this exercise as homework. Short and concise, it allows me to evaluate their knowledge of music on their own time. Questions cover scales, clefs, key signatures, rhythm, intervals, and chord inversions (triads and 7ths) and should take about 30 minutes. Instructions at the top dictate, "fill these staves to the best of your ability." Aimed at 9th grade and older.
